 Access to your Medical Information

 

Summary Care Record

 

This is an electronic record containing information relating to any medicines you may take and allergies you may have including any reactions to medication. Having this information on this record enables any Health Care Staff to be able to treat you appropriately when your Surgery is closed.

 

 

Detailed Coded Record

 

NHS England also requires us here at Park Lane Surgery to allow you access to a more detailed version of your Summary Care Record. This includes test results, referrals meds from us and letters recieved by us, and codes relating to medical/surgical procedures you may have had.

 

There will be some circumstances where your GP may feel seeing this information may affect your physical and mental wellbeing. In these cases, they will be able to withhold some coded information in the interest of the patient and also where there is a reference to a third party.

 

While we at Park Lane are committed to this system, we would like to stress that these records are meant to be used for Medical Professional to be able to offer you the best possible care. Some of the information may be difficult to interprept due the technical way it has been written by the specialist.

 

If you would like to have access to your records in this way, please request this by again clicking Online Services tab. You will be required to provide photo ID and complete a consent form.
